Way too many hoisting system failures in this video. Either operator failure or lack of inspection or replacement of the wire ropes.

Here is a video I have seen at safety meetings in the past. I have tested thousands of these parts @ 1.5 times the cold working pressure. A 15000 cold working pressure part is tested @ 22500 psi(hydro test) before being sent into the field. These guys work around this stuff pressured up every day. They earn every cent they are paid. No way I want to be near all of that pressured up iron. I have had it fail before during a hydro test and it will completely rearrange the test room.

Dangerous place. Off the top of my head I can quickly think of six young men who died in oilfield accidents. One a nephew who died when a well torpedo blew prematurely.

Hell to the no, they don’t pay near enough for me to play in that sand box. I hurt myself on dry ground, I’d be dead inside a month doing that.

A couple of guys from my hometown were killed in drilling rig accidents. Also, an oil refinery is a dangerous place. Being a Process Engineer, I was assigned different process units over the decade I was in the plant. The more experienced I got, more complex units were assigned to me. One was a Hydrocracker, operating at over 2000 psi in some parts of the process. While I had the unit, the Senior Operating Foreman was fried when a high pressure furnace malfunctioned. One other time, during lunch, when all the operators were eating, I walked through the pump section. A pump seal had blown out and very hot smoking feedstock was running across the concrete and down into the rainwater drain. It didn’t flash and burn. It would have been a disaster. And just after I left to transfer to Houston, a distillation tower accident and explosion killed 13 of my former coworkers and friends.

A rule of thumb back then was if you saw people running, don’t ask questions. Just run with them. If they are climbing the fence, climb the fence.

My nephew had a sucker rod go thru his shoulder and exit around his hip.Didn't kill him then but his working career was over for two years before he passed.

One night it was raining like crazy and we were going back in the hole.Driller had it cranked wide open and counting the wraps on the drum and Ernie (derrick hand) had pipe in hand when blocks went sailing by.He hollered and let go of the pipe unbuckled his safety harness and jumped out of the derrick on the safety rope.About the same time the driller let off the throttle and slammed on the brake and I took off running.I heard the blocks hit the crown but I kept running.By the time I got back everything was good and after checking we started back in the hole again and I almost quit that night.

I did notice that they used European dating with the day first then month, I also noted what appeared to be Cyrillic charters on some clips, I'm thinking that most of this occurred in Russia.

Pumping energized fluids is always as dangerous as it gets. We pump a lot of nitrogen jobs and Nitro Cement jobs too, never without the endless loop restraints. Even with the restraints, it will just minimize damage, a failure will be catastrophic.

I have seen a few nasty accidents, the worst over by Eagle Mountain Lake on a drilling rig during a casing run. We were waiting to rig our cement crew up when there was a horrendous noise. They were using cable chokers to pick up casing, and the stabber in the derrick would latch the elevators when the casing set down on the stump. Something happened and the driller let the choker come down too far on the casing, and when he put the draw works in gear and mashed the throttle the casing picked up way crooked and spun around and the collar came up and hit the stabber just under the chin and killed him. We were locked in there on that location for a long time, and it was super eerie getting on that rig floor and rigging up the next day. Two years prior to that on the same rig, we rigged down our head, and the rig hands were removing the spider slips from around the casing. They didn't close all the way, and got caught, so they picked them up a little, they re opened, and one of the rig hands decided to push on it with his heel and he slipped about the time they closed and basically mashed the muscle behind the shin bone to a pulp. He screamed, hobbled down the stairs and passed out on the ground. I saw him a year or so later, and he made a full recovery, but his leg looked pretty botched up. He was a lucky one.

I worked offshore for a couple of years after HS, production platforms and a couple of drilling rigs, never worked on the drill floor thankfully. I've never seen the block falling like that and I've never seen the table come up like those scenes show. Very scary stuff...

I once saw a safety video where they had set a bunch of crash test dummies around a perforating gun on the racks and set it off. The carnage was incredible. The biggest piece left was the size of a quarter. In my early years, i got to a rig in NM just after the driller had run the blocks into the crown and cut the drill line. Thankfully no one was hurt, but there were a lot of ghosts faced hands walking around.
