What happens if your company that you are no longer employed does not send you a income statement. They did not even send me monthly statements.

Employers are required by law to send your W2 by January 31.

You can create your own W2 showing your estimated earnings and taxes with held. You just attach a note stating why you had to do so. The IRS will followup with the company to obtain a true W2 and adjust your tax return accordingly.
