From what I have seen the milled is a little nicer, it really depends on the manufacturer though. What are you looking at?

Typically your Lowers will be forged, not cast, and forged parts will be inherently stronger(this is really a non-issue, though). Your billet lowers will have cooler designs and features, but will cost more. You will see stuff like the one below, which runs about $250: Your forged lowers, in most cases, will be the typical stripped lower, as the forgings are cheap and readily available, where making new forgings(IE: Noveske Flared Forged Lower, second pic, These also run about $250.) is quite expensive, so you tend to not see those as much. Really, it comes down to personal preference.
