These incidents are a inversely proportional to thorough properly timed maintenance. Predictive maintenance on this type of equipment eventually results in failure. But the bean counters like it until something bad happens. I've been pushed into those corners, but would not play ball. I believe reliability and safety go hand in hand when dealing with high voltage and wouldn't accept anything less.
