I believe the Glock 40S&W issues come about primarily when shooting lead bullets. I've rarely heard of an issue when shooting factory ammo (or non-lead bullet reloads done to spec). Nevertheless, the "Glock grenade" myth lives on.

Nothing wrong with lead in factory Glock barrels. I had around 12,000 of my hard-cast bullets through my gen 3 G35 before I sold it, and I cleaned it minimally. Also, I only shoot range brass and never had a problem with the "Glock bulge" in the aforementioned G35, XDm, or my Para 16-40.

As far as the OP is concerned, I would bet on bad brass.

I'm not really qualified to add much to this but one thing came to mind. Any chance of the wrong powder being in the trickler and this is the first round with the wrong powder?

I would definitely tear down the rest.

Just a follow up to my old post. Well this exact thing happened a second time. Turns out, all of my old brass I loaded with the unique powder well all over pressured with the load data from Alliant's website. 6.7gr's. Made the brass weak. Then it was just a matter of time before one decides to let go. Thus far I have disassembled approx. 200rnds of potential bad brass and put it in the scrap pile.

As far as the glocks bulging the brass. My friend's glock does it. When the brass is resized it is straitened out. But I get some of his by mistake, I only load it 1 more time before I scrap it as well.

I do no reload for .40, but I have had two factory loads from the same lot separate at the case and blow the extractor out.

Agree with this. Either the round did not initially fully chamber or backed out upon ignition; how else can the brass head/rim get that kind of specific malformation? The left hand side of the case head looks like it flowed onto the loading ramp.

I have a G22 and a G23 and a few years ago I was shooting one or the other and had a chunk blown out of the case right next to the extractor groove. Blew my magazine out and recoil and noise were more than normal. I always laid it on too much powder, but after reading some of these posts, now I'm not sure. These were gun show bulk reloads also.

The round was fully seated in the chamber. It is a XD40sc. The striker will not release until the slide is fully closed. Being that has happened twice with the over pressured brass I would safely say that it is a brass problem.
