Fast food chain wise: I have to go with OC burger, they have some seasoning that is just really good to me.
Until a couple of months ago, I really loved Ted E's burgers in Ft. Worth/Keller off of Basswood and Beach St. However there is a strip club called Pin Ups on Mark IV parkway that had one of the best cheese burgers I have ever had. Of course that could have been because all of the entertainers left a lot to be desired but I do swear the burger was extremely tasty.
If dueling were still legal I think there would be less idiots in charge of things...

Ok, I think that's officially the first strip club burger nomination on this thread.

If you are ever in the Paris, TX area if you go just a little further north into Hugo, Oklahoma there is a little cafe called the Busy Bee Cafe. I haven't been there in years, so it could have closed, but I love visiting my aunt and uncle there just for that reason alone.

Stackhouse Burgers on Gaston in Dallas. I'm in the meat business, trust me. This one's the best I've had anywhere.
