The cylinder stop is not engaging. Could be a broken stop, could be a broken spring, could be dirty or reassembled improperly.

More than likely it's the cylinder bolt spring.
That spring pushes upward on the bolt so that it engages the notches on each cylinder for alignment.
It could be the bolt itself is broken, but far more likely it's just the bolt spring.
