How mistakes are made when identifying the target always made for good discussion in my Hunter Education classes. I would start by first showing the class a photo out of the blue and asking is someone could find the deer in it. I would say something like, "Okay, let's see who has good eyes. Who can be the first person to locate the deer in this photo. It never failed that someone (and often several people) would raise their hands to claim they could see the deer. Of course, there was no deer in the photo, so they were tricked of sorts. Still, it made the point that if the human mind tries hard enough, it can see a deer where there is none.
Short of a criminal act, these accidents point to how people, who for one reason or another, truly believed they saw a deer or at least they "thought" they saw a deer. After the photo exercise, we would move into a group discussion on when a person knows without any doubt they are looking at a deer, rather than just assuming they see a deer.

I had a bruiser of a 10 point branch about 50 yards to my left before legal shooting hours Saturday morning. Of course I would not shoot without positive ID and legal hours but your eyes and mind can play tricks for sure.

Wanting to kill something so bad you loose judgement. Temp insanity!!! As a 12 year old I almost shot my dad swinging on a running pheasant. Scared bejesus out of him but he was a great Dad and we talked it through. As an instructor this is my most important real life discussion. You Must not loose control. I tell them I pass on more shots then I take due to caution

These are all tragic, no deer or bird is worth these outcomes.
