Release form for underage person

I have been invited on a 4 day hunt in S Texas at my friends lease. She told me I could bring my nephew with me with is 13 years old. With that said, she asked that I get a release form signed and notarized from my brother giving me the power of attorney over him just in case something happens and I have to sign papers for him if he gets hurt.

Does anyone have a template form I can use for this?

Should be able to find one on the internet. Templates are easy to come by. However this is a really good idea that gets overlooked 99% of the time.
