4 to 6 bucks a sq foot is a good price to go on that I have always seen.

No one I've heard of is placing concrete for that price - if they are they can't be making any money or they are desperate for work. At 6", it's almost 3 yds of concrete, which will cost you $400 in mud alone. At 4" it's 2 yds which will cost you almost $400 as well - that's right, no one right now is real keen on delivering much less than 3 yds unless you use one of those tote it yourself jobbies from a small batch plant. I would also hammer out that rectangle and place it all in one piece. I personally would never lay a 4" pad for a driveway although I know people do it all the time.

You probably have a 4-6' setback with that fence. You may not care however but if you get called on it you can deal with it later. Looks like you're interrupting the natural drainage so I would make sure you reshape the dirt in front of the pad and create a swale between your drive and the fence or even put in a catch basin and drain pipe for any water that flows out the to the street otherwise it will undermine the corner of your pad.
