My dad has a wilson combat barrelled ar-15, ever since he had it, he will drop a shell in the chamber and hit the release, then he will put the mag in. We'll this time the ar-15 fired when it slammed home. Luckily he had it pointed down into the ground. The casing on the left is from the shot. Im guessing maybe soft primer and the floating firing pin along with the bolt not being slowed down by the magazine. He has done this since he had this rifle going in two hears, but has had several and has done the same. I've heard of ar-15 doing this but have never actually knew someone it happened to. Pnw factory ammo. Are my thoughts in line on what happened.

Slam fire. Proud primers will increase the chances too. You should check the bolt lugs for damage as it was probably only partially in battery. Early on I was told not load a military firearm with a floating firing pin in this manner. Not calling anybody out, but if you insist on chambering the round first ( typically done when shooting long range with COAL greater than mag length) you should partially ride the bolt down.

Let me add one more thing your photo shows, the firing pin was in rebound so the primer metal swaged into the firing pin hole with no resistance from the pin (the hammer was not there to hold the firing pin forward). I am wondering if upon closer inspection you can see a little dimple in the center of the primer from the pin hit. It may have been completely fire formed out though.

So, the full momentum of the bolt hit the case, without being slowed down by picking up and chambering a round, and there is no spring to help hold the firing pin back?
Besides the stress this puts on the bolt and extractor...

Primers not fully seated can do this as well
