That's a big knife, and a tough grind. On the other hand, the handle fittings are relatively simple. Price depends on who you get to do it and what level of fit and finish you expect. I wouldn't touch it for 600, might for 8. You might could find it as low as 3 or 400 from the right guy. I'm not taking orders, just speaking hypothetically.

It appears to be carbon steel, probably about 3/8 thick and it looks like the carbon steel was used for the guard and the rear bolster?

It looks hollow ground to me. I'd suspect ground over forged,but you never know. It's all grinding in the end smile

It would be much cheaper to have it made than what it will cost you to buy all the equipment and materials you will need.

I would think that it needs to be forged to get the correct elongated diamond profile before hollow grinding. I know Jerry Fisk meets with a number of young bladesmiths perhaps he could recommend someone.
